Madhepura UDID Disability Certificate

The Unique Disability ID (UDID) is a national card and certificate issued to persons with disabilities, verified by competent medical authorities, that simplifies access to government benefits and schemes.

How to Apply

1. Visit swavlambancard.gov.in and select "Apply for UDID Card."
2. Enter personal, disability, employment, and identity details.
3. Upload a passport-size photo, Aadhaar details, and proof of address.
4. Attend medical verification/assessment as scheduled to confirm the disability certificate.

After Applying

You can track your application status and download your e-Disability Certificate or e-UDID card directly from the portal once approved.

Why It Matters

A UDID card is often required to access disability pensions, reservations, and other welfare scheme benefits — including the Madhepura disability pension.
